    The PUK code is specific to sim cards. The way to get that code is by calling customer service. If it is verizon wireless, the number is (800) 922-0204 when you hear the recording press #0#0 and that will transfer you directly to a live person.

    While the PUK is specific to each SIM card, you can retrieve this without calling Customer Service if you are registered for My Verizon. If you have a My Verizon account, simply log in as the account owner, select the number that has the Galaxy S II, and hover your mouse over the My Verizon heading. From the drop down menu, click on View All in the My Device section. From this page, you can click the Unlock SIM (PUK) Now link, located to the right of the picture of your device. The default Pin and the Personal Unlocking Key (PUK) will be displayed.
