Are you referring to Verizon Fios equipment? If so I would recommend instead taking it to a local UPS. You don't need to package it or an address. Just ask them to return it through the Verizon Fios account. Make sure you get a receipt and keep it for 90 days at least.

As long as you have the receipt, the box and everything it came with (unless it is a warranty exchange them just take the phone) AND it's an actual Verizon store not a reseller you can return it to any store.
If you purchased it at an independent location (the receipt might say The Wireless Connection, Tower Wireless, Etc) and include the words AUTHORIZED RESELLER, Independent store, Preferred Wireless Partner then you would need to return it to one of that company's locations.
If it is Verizon Wireless equipment, it can be returned to any corporate store with 14 days of purchase, as long as it was purchased directly form Verizon Wireless and not an indirect agent or premium retailer. If you are returning a handset for anything other than a manufacturer defect and exchanging for the exact same model, you may be required to pay a $35 restocking fee.
