I've accidentally locked myself out of my cell phone and a PUK code is being requested.

I've accidentally locked myself out of my cell phone and a PUK code is being requested. I don't know who my provider is (I think it's T-Mobile) so is there any way to find out this info and enter my PUK code into my phone to unlock it?

If its your passcode then you want to enter the code in wrong till it asks for a PUK code and The PUK code is a lock code for your sim card. That happens by entering too many wrong patterns/pass codes. All you have to do is either call customer service at 8009378997 or go into a local corporate store with your ID and tell them you need your PUK code. Each phone number has its own PUK code so this is the only way to get it. Customer service will give you the PUK code & walk you through it step by step. If you go to the store they will do the same but just make sure you are the account holder/authorized user. IF you’re not them have them with you. It’s a fast and easy process.

all you have to do is either call customer care at 1-800-937-8997 or go into a corporate store with your ID and phone (granted you are the account holder or an authorized user) and request you PUK code....each number has its own PUK code so there's no other place to get one....they will give you the code and explain how to put it in....takes minutes and is free

Again, I'm not sure which carrier I have. Is there any way to find this out? Or are you saying I can go into ANY carrier's corporate store and request my PUK code?
      Thanks.

Sorry...my bad! I just found some manuals in a drawer (for all the phones in our family), and the one with my cell phone # written in pencil leads me to believe my phone is a Samsung SGH-a197, and the carrier is att. Please disregard the rating I just gave you. You were indeed very helpful. Let me know if there's a way to change the grade I gave you.
      Thanks!
