The best way to do this is to either call customer service and let them know you want to submit feedback about a store representative. They have a form that they can fill out for you and submit to that store's direct supervisor. Another way is to call the store directly and request to speak with the store manager. You can also visit www.vzw.com/contactus and follow the steps under "send an email" or send a letter to the corporate mailing address which will go directly to upper management.

By going to www.verizonwirelesssurvey.com, your comments get tagged to the particular store/sales rep you dealt with at the location. Any surveys entered from this site, along with the comments you leave are forwarded to the retail store's management AND forwarded up the chain to the district manager and then the director of sales for that area. This would be the quickest way of voicing your experience to someone who listens. I would also expect that you would receive a call back from management depending on what your survey comments include.
