    To unlock your phone you will need your imei code by typing *#06# on your T-Mobile phone. Wright down the numbers & call customer service @ 8009378997. Tell the rep that you need an unlock code. They will ask for the imei and your email address. You should get an email in 48 hours. The email you get will contain unlock code and instructions on how to use it. Now if they don’t get it to you then there are other ways such as ebay, cellphoneunlock.net, cellunlocker.net, unlockitnow.com and there might be a local repair shop in your town that will perform this as well.

      For the PUK code just call customer care @ 8009378997 or go to your local store. The PUK is a unique code that goes with the phone. You can only get it from a T-Mobile rep.

    all you have to do is either call customer care at 1-800-937-8997 or go into a corporate store with your ID and phone (granted you are the account holder or an authorized user) and request you PUK code....each number has its own PUK code so there's no other place to get one....they will give you the code and explain how to put it in....takes minutes and is free
