Remove and reinsert the card.
Make sure the card isn't inserted upside down or askew.
The chip should be facing down with the logo or picture facing up.
The arrows should be pointing in toward the receiver, if it has arrows (not all cards do).
if still having the same error try:
Reset the receiver without the access card.
Remove the access card and reset the receiver without the card inserted.
Be sure to reinsert the access card when done.
If still an issue - either a problem with the card or the reader; you'll need to call into technical support at 1-800-531-50000 comments1 other expert is backing this answer

If that doesn't work, your access card reader may be out, you'll need new receiver then. You would call 18005315000

Sometimes the information between the card and receiver gets mixed up. This is usually when a receiver has not been used for a while, but can happen anytime. You can call 1-800-531-5000 and they will resend authorizations which should fix the issue. It's quicker and easier to do on www.directv.com. Sign in (or sign up, it's easy and REALLY useful), and in the help box type '721'. On the results page you'll see a link to 'resend authorizations'. Goes through immediately, where when done on the phone can take up to 24 hours!!
