    Bereavement fares are available only by calling Delta Reservations at 1-800-221-1212. Medical Fares are available when traveling within the 50 states, Canada, Puerto Rico and the U.S. Virgin Islands.

    Required Documentation
    The following information is required so that you can purchase a ticket using Bereavement Fares:

    Deceased person’s name
    Passenger's relationship to the deceased
    Name and phone number of funeral home, hospital or hospice
    Name of doctor (if applicable)
