If the phone is no longer asking for a PUK code, then you will have to replace your SIM card as it has been disabled. You can stop by an AT&T Company Owned (COR) store for a free replacement. If the phone is asking for the PUK code, you can login to our website at att.com and search for "PUK Code". If that does not work, call customer service and they can give you the code as well.
