    Hi Johnny -

    You have a few options available to you to recover your PUK code.

    From the Online Account Manager (which requires you to have a registered account) Get the PUK Code to Unblock Your SIM Card (https://www.att.com/olam/passthroughAction.mywo...
    Go to the My Phone/Device page in myAT&T.
    From the My Phone/Device screen, select the Unblock SIM Card link under Manage my phone/device.
    The Unblock SIM screen will display your PUK code and will give you instructions for unblocking your SIM Card.


    Do not attempt to enter a PUK code unless directed to do so by a Customer Service or Technical Support representative. Entering a PUK code incorrectly more than 10 times will cause the SIM card to lock permanently.

    To get the Puk code you can go to the AT&T wireless store and they can do that for you at no charge. Thank you and good luck!

    super anonymous is correct, the best and easiest way to get your PUK code is to go directly to any att corporate store and a rep can get that for you in a matter of minutes. make sure when they reset the puk that you enter a new pin for yourself. this will be the new unlock pin for your phone. hope this helps!

      if you are able to log into wireless account online then once in the acct overview click on the "I WANT TO..." and there the puk code link is available for you to use at anytime. and mr attack is correct put in a pin that is easy to remember as anytime the phone gets powered off for first few times after the pin code will be requested and it will be the one you create.
